The Community Based Palliative Care Nurse Practitioner works in conjunction with the Community Based Palliative Care Physician, Direct Care Case Manager, and Community Health Assistant, to provide a team approach in caring patients in the last 6-12 months of life who are enrolled in Geisinger at Home. The NP will provide direct hands on quality care in the patient’s home. The NP will develop care plans and interventions to meet the patients care needs, while connecting with the primary care provider. The NP will report to the Chief CRNP as well as the Regional Medical Director and Director of Clinical Operations and Palliative Physician.

Education and or Experience:
The successful Nurse Practitioner candidate will have a certificate of completion from an approved program for Nurse Practitioners and must be eligible for or certified by the Pennsylvania State Board of Nursing.
The successful Physician Assistant candidate will work under the responsibility and supervision of the physician and require completion of an ARC-PA (or predecessor organization) approved physician assistant program, current NCCPA certification or eligibility and Pennsylvania State licensure.
Previous experience in end-of-life care preferred.
Minimum two years of clinical nursing experience required.
The successful Nurse Practitioner candidate will work in collaboration with the physician and have a certificate of completion from an approved program for Nurse Practitioners or be eligible to hold national certification that allows the NP to work with the patient populations and location settings associated with this position. Must be eligible for licensure as a nurse practitioner in the state of PA.
Working Conditions and Physical Demands:
Ability and willingness to travel.
Work is typically performed in the patient’s home.
