The Nurse Practitioner (NP) Physician Asssitant (PA) works in conjunction with the Home Care Physician, RN Case Manager, Social worker, and Pharmacist to provide a team approach in caring for the most complex patients. The NP/PA will provide direct hands on quality care in the patient’s home. The NP/PA will develop care plans and interventions to meet the patients care needs, while connecting with the primary care provider.

Demonstrates the ability to be a self-starter with ability to handle multiple projects at one time.
Demonstrates excellent organizational skills, well organized, efficient and able to work under pressure.
Demonstrates customer service experience and skills.
Demonstrates strong written and verbal communication skills which will meet standards of practice and regulatory requirements.
Demonstrates strong interpersonal skills to include: effective listening skills, effective management/leadership skills, effective teaching skills.
Education and or Experience:
Must have certification of completion from an approved program for Nurse Practitioners/ Physician Assistants and have or be eligible for current license in the Commonwealth of Pennsylvania. Must have BLS and have or be eligible for ACLS.
Prior experience as a Hospitalist or Critical Care Nurse Practitioner / Physician Assistant preferred.
Working Conditions/Physical Demands:
Ability and willingness to travel.
Ability and willingness to take call after hours and participate in weekend call rotation.
Work is typically performed in a the patients home.
