The Liver Transplant Coordinator performs in a specialized area of nursing that focuses on managing a caseload of outpatients and inpatients through close coordination of care with multiple disciplines focusing on discharge planning needs and self-managed care. The Liver Transplant Coordinator coordinates care for Veterans with complex, high risk conditions requiring various levels of care. The Liver Coordinator functions in an expanded role requiring comprehensive knowledge of hospital systems, community resources, ability to work well with patients and families in stressful situations and ability to advocate for the best interest of patients. Duties may include but are not limited to: Demonstrates leadership in delivering efficient, comprehensive, and patient driven holistic care through collaborative strategies with others. The proficient nurse learns from experience what typical events to expect in a given situation and how plans need to be modified in response to these events. The RN Liver Transplant Care Coordinator possesses intimate knowledge of InterQual criteria, utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines. Demonstrates knowledge and ability to apply principles of evidence-based practice. The Liver Transplant Coordinator is responsible for the provision of high quality, patient-focused and cost-effective patient care as it relates to his/her role. The Liver Transplant Coordinator works in partnership with Hepatology and Gastroenterology providers and identified patient populations for effective disease, chronic care management, liver transplant pre-evaluation and post-transplant management. Maintain documentation of care provided in CPRS and other appropriate resources Maintain databases and be familiar with computer programs CPRS, outlook, excel, Microsoft office to analyze data. The Transplant Coordinator provides direct and indirect care to include assessing, planning, health promotion, preventive care, education, research activities, and consultation Works collaboratively with VA and non-VA transplant centers and manage referrals Utilize critical thinking skills and sound judgment to ensure positive outcomes and deliverance of quality patient care. The incumbent will be responsible for initiating and participating in performance improvement initiatives and assist with collecting, monitoring, and maintenance of relevant data. Assist in scheduling patients for outpatient clinic appointments, procedures, and necessary testing as directed by service protocols and plan of care Other duties as assigned in relation to this position Work Schedule: Monday - Friday 7:30 am- 4:00 pm or 8:00-4:30 pm. Due to the nature of this position, it requires flexibility in schedule and assignments. Extension of TOD may be required as some work may need to be completed during non-clinic hours.
