This position is located within Thunderbird CBOC (Community Based Outpatient Clinic), Phoenix VA Healthcare System (PVAHCS), Phoenix, AZ The TCT-CBOC LPN serves as a CBOC-specific clinical facilitator of Telehealth services, assisting Veterans and VA clinicians with successful implementation and expansion of virtual care modalities. This is not a telework. Performs a full range of duties and is responsible for day-to-day operations of telehealth services at the assigned CBOC and other remote sites of care. The incumbent is expected to exercise independent judgement and will often provide guidance to less experienced staff members and will require minimal direct supervision. This advanced position functions above the full performance level and therefore requires the performance of higher-level duties which must consist of significant scope, complexity (difficulty) and range of variety and be performed by the incumbent at least 25% of the time. Assists with special projects or other duties as assigned by FTC/Nurse Manager. Assignments associated with this position include, but are not limited to: Provides advanced clinical technology information to patients, staff, and providers regarding Telehealth, Education and Training processes. Assists remote and local providers, employees and patients as needed. Has regular contact with the facility telehealth director to work out any process issues, equipment needs, problems, data collection, and any other logistical issues. Creates the appropriate clinical and/or training setting/environment using interactive technology and is knowledgeable of adult learning using interactive technology and maximizing environments to support such activity. The above activities are conducted for Clinical Based Telehealth and Home-Based Telehealth. Responsible for any patient, staff education/training, documentation, and assistance with workload capture that is needed for completion of visits and training. Coordinates information between facilities to expedite patient Telehealth modality (SFT, VVC, & CVT) to access, promote and ensure maximum of continuity of care intra- and inter-facility. The incumbent is the point of contact for both outpatient and inpatient referrals requiring Telehealth services and telehealth assistance. Provides cross coverage for other TCT-CBOC staff out on leave either remotely or physically to ensure patients received timely care Maintains coordination and consolidates the Telehealth schedule at the assigned CBOC to assist in multiple appointments of patients using VA Telehealth equipment/ technology and other resources to address our wide catchment areas of Telehealth services. Maintains daily Telehealth schedule at assigned CBOC to assist in scheduling rooms, patients, technology and tele-presenters. Identifies and recommends ways of eliminating, combining, simplifying or improving procedures and processes of the Telehealth program. Performs Telehealth imaging, including tele-retinal screening, tele-dermatology, tele-wound care imaging and other Telehealth programming as technology becomes available, as identified and referred by the primary care provider, other providers, and the imager. Guided by local policy/standing orders, the imager can initiate consultations for patients on behalf of the primary care provider, who retains responsibility for patient care. Provides patient education regarding the imaging process. TCT-CBOC LPN may identify and access patients via VISN data systems. The TCT-CBOC LPN will generate a consult and appropriately appoint the patients. Images will be obtained using approved Telehealth imaging devices. The images will be placed in the patient's electronic medical record via CPRS and VistA Imaging. For retinal imaging the TCT-CBOC LPN will obtain multiple digital external and non-mydriatic retinal images. The TCT-CBOC LPN will document all clinical information in the electronic medical record (CPRS). The TCT-CBOC LPN will ensure encounters and documentation is complete/thorough/accurate as to avoid open encounters and orphans. The TCT-CBOC LPN runs regular EARR/Orphan reports to evaluate for open encounters and orphans and assists in the resolution of them. Post-exam patient education will include such things as: review of the acquired images for anatomic and general findings, discussion with the veteran of the link between blood glucose control blood pressure, and eye health, review of photos illustrating non-proliferative and proliferative diabetic retinopathy and retinal laser treatment, follow up visit scheduling/ guidance, Work Schedule: Monday to Friday 7:30am to 4:00pm Financial Disclosure Report: Not required
