Continuing education (CME/CE credits and MOC points) is available for a variety of enduring materials on the portal. All current AASLD members automatically receive complete access to LiverLearningÂ®, and non-members can purchase access for one year with a subscription.
Enhance the organization’s mission by delivering diabetes education through an ADA Education Recognized Program (ADA ERP) for patient education. Provide educational classes to patients of The Villages Health and conduct community education sessions as requested. Act as a resource on diabetes management to clinical staff.
Provide both individual and group diabetes education programs following the curriculum established for the ADA ERP certification
Conduct initial assessments to provide education, coordinate resources, and determine need for diabetes education programs
Responsible for documenting patient assessments and progress in electronic medical record and other databases
Set up education area for class sessions and break down materials when complete
Act as a diabetes education resource with TVH clinical staff and at community events
Maintain patient care schedule
Provide community education services through facilitation of Diabetes Prevention Program and Learning Center seminars
Provide regular reporting to Supervisor on diabetes education program effectiveness
Troubleshoot problems and develop recommendations for resolution within the unit

Must be extremely organized and flexible to adjust to multiple task requests and changing priorities.
Proven experience as a self-starter who can work with little direction.
