The Licensed Practical Nurse (LPN/LVN) assigned PACT/VICTOR teams at OPA 2, provides safe, individualized and integrated holistic practical nursing care to patients who are acutely and/or chronically ill in the Outpatient care setting, such as renal, cardiological, neurological, pulmonary, GI, surgical, dermatological, orthopedic and endocrinologic conditions. Care is provided to patients who exhibit a variety of physical problems. Vacancies exist in the Patient Aligned Care Team (PACT) for dynamic and creative practical nurse with an excellent grasp of clinical practice who would work to support the Primary Care Clinic- Patient Aligned Care Team model. The individual selected for this position is accountable to the Nurse Manager in the ambulatory care setting/Primary Care/PACT and works under the direction of the Registered Nurse (RN) and within identified teams. Responsible for the treatment, observation and care of the Veteran within the scope of LPN functions and other duties as assigned. Assist in the overall PACT clinic management, functions and operations, facilitation patient flow, thorough check in and checkout process. Provide patient education and/or instructions. Work with care team and community resources. Participate in performance improvement activities and other activities that enhance patient care and clinic operations. Maintain patient's privacy and confidentiality. Document pertinent information in the electronic medical record. Assist with coordination of all services for the patient with PACT Team and group visits. Work with the patient, significant other and health care team to establish patient centered short and long term goals. Demonstrate knowledge and skills sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications/vaccinations as permitted by approved local facility policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any noted change in patient's condition to the Registered Nurse. Work Schedule: Monday - Saturday Day Shift Extended Hours
