Purpose Statement / Position Summary
The Registered Dietitian will be an integral part of the multidisciplinary team known as ACTIVE diabetes in which they will collaborate with physicians, clinical pharmacists, and social workers to manage diabetic patients not at their A1c goal, despite traditional management from the primary care and or Endocrinologist. Eligible patient populations include HMO, ACO, PPO, and Medicare. The expectations regarding patient care and follow up will be guided by protocols developed by the ACTIVE diabetes based on current evidence-based medicine. The position may also provide opportunity to manage newly diagnosed Gestational Diabetics.
Essential Functions and Responsibilities of the Job
- Current knowledge of nutrition therapy and skills in educating individual and groups of identified patients with a focus but not limited to diabetes education and management.
- Work collaboratively in a team based approach within the Collaborative Health Center and Diabetes Care Team.
- Ensure appropriate and cost effective solutions for patients in need of nutritional guidance.
- Adhere to evidence based standards for nutrition and dietary guidance.
- Contribute, develop and complete the maintenance of educational documents, policies and workflows that support Memorial Care Medical Foundations Dietary and Health Education services.
- Contributes effectively to the overall function and organization of the ACTIVE Diabetes for Memorial Medical Foundation

- Create menu examples and appropriate materials when needed and evaluates patient’s understanding of material.
- Work collaboratively in a team based approach within the Collaborative Health Center and Diabetes Care Team to ensure and achieve the Triple Aim.
- Document all activities correctly and timely based on best practice
- Triage and develop best practice approach for high risk Diabetics within the Diabetes Care Team
- Provide timely access to patients within the CHC/DCT
- Work alongside colleagues to ensure proactive and innovative approaches and in achieving lean workflows to maximize results.
- Communicate and Interact with internal and external customers in a positive, courteous and professional manner
- Ability to prioritize and multitask effectively with appropriate notification to manager
- Adhere to evidence based standards for nutrition and dietary guidance and updates annually or as needed.
