The Registered Nurse - Cancer Navigator - Pulmonology serves as reference outpatient procedure areas to facilitate timely, appropriate work-up of newly complex cancer cases in collaboration with the providers assigned to their specialty area. This position is located within the North Texas VA Health Care System in Dallas, TX. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Education: Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. License: Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: At least 2 years of experience working in: Pulmonology, Gastroenterology (GI/Liver), Dermatology, Hematology Oncology, Dialysis, & Genitourinary Specialty certification related to one of the following specialty areas: Pulmonology Registered Nurse (CGRN), Oncology Experience and knowledge of antineoplastic agents - Pulmonology Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Physical Requirements: Lifting up to 35·pounds for 8-12 hours, intermittently, moderate carrying (15-35 pounds) for 8 to. 12 hours, intermittently, lifting over 35 pounds with assistive devices in accordance with the Safe Patient Handling Policy except in emergent situations requiring prompt patient care, pulling for 8 to 12 hours intermittently, pushing for 8-12 hours, intermittently, reaching above the shoulders for 8-12 hours, intermittently, both hands required, walking 8-12 hours, intermittently, standing 8-12 hours intermittently, kneeling 8 to 12 hours, intermittently, repeated bending for 8-12 hours, intermittently, climbing and use of both legs and arms for 8-12 hours, intermittently, both legs required, near vision at 13 to 16" Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other eye, ability to distinguish basic colors, hearing aid permitted, twisting, simple grasp and fine manipulation. The Nurse Cancer Navigator (NCN) supports VA strategic goals to improve access, provide reliable, integrated care and to improve business processes. The NCN aligns closely with VA priorities and initiatives to improve customer service, suicide prevention, whole health, and high reliability efforts. The Nurse Navigator will work with other clinicians and Services to identify barriers and improve cancer care coordination. Possesses strong inter-personal and communication skills in written and spoken English. Ability to work in a self-directed manner, collaborating and coordinating with health care professionals across the organization Diagnosis and Treatment Works collaboratively with the residents, fellows, mid-levels, and/or attendings to coordinate work-ups that optimize VA resources, facilitating community care as needed. Serves as point of contact for any work-up related questions from patient/family, staff (local and/or other VISN17 facility), community providers. Informs patient/family of process, general disease education Tracks diagnostic imaging and pathology reports to conclusion, assuring timely review by specialists Ensures timely Tumor Board review in collaboration with interdisciplinary members, gather care plan from Tumor Board, and support communication of plans to residents, fellows, mid-levels, and/or attendings and patient/family. Monitors activities to ensure timely multidisciplinary review and coordination of activities necessary for development of a treatment plan and necessary supportive procedures (e.g. placement of access/IV lines, dental extractions, etc.) Assesses and link patient to additional services when needed, i.e. alcohol treatment, nutrition, palliative care, hospice as necessary, psychosocial distress screening Communicate/coordinate with residents, fellows, mid-levels, and/or attendings re: medications, patient care needs, etc. Supports patient education, treatment education, awareness of steps in care Assesses and coordinate treatment for symptom management, and/or refer to appropriate specialty for higher level care needs OPCCNs for Liver conducts FibroScan testing Active Treatment Tracks all treatments according to care plan, communicating with appropriate service provider on any deviations/omissions from plan For patients receiving community care, closely follows care plan to assure adherence with VA authorizations; collaborate with community provider to maximize use of VA resources, and assure timely transition back to VA providers when appropriate Assesses and coordinates treatment for symptom management, and/or refer to appropriate specialty for higher level care needs Transitions in Care Assures treatment summary and recommendations are communicated to involved services Works collaboratively with clinical services (VA and community) for timely scheduling of all follow-up care Provides education to patient/family for medication changes, symptom management, etc. Assesses and link patient to additional services when needed, i.e. alcohol treatment, nutrition, palliative care, hospice as necessary Supports patient education, treatment education, awareness of steps in care; act as health coach, proactively supporting patient to optimize treatments VA offers a comprehensive total rewards package: VA Nurse Total Rewards Work Schedule: Monday - Friday, hours (0730-1600, 0800-1630 etc.) and subject to change as needed to meet patient care and clinic needs.
