The Patient Care Programs Coordinator coordinates and supports individual patients and families throughout point of care program(s) to ensure program goals are effectively executed with some guidance. Assures coordination of diagnostic evaluations, treatments, surgeries and follow-up appointments throughout the course of care/episode for a specific patient population.
Vanderbilt University Medical Center Transplant
The Vanderbilt Kidney Transplant Program is one of the oldest and largest programs in the country. We offer innovative treatment options for kidney diseases in addition to kidney transplantation. Our specialists also perform lifesaving dual-organ transplants for patients with liver-kidney dysfunction or heart-kidney dysfunction. We have performed more than 6,000 pediatric and adult kidney transplants since our program started, giving us deep expertise with even the most complex cases.
Key Responsibilities of the Patient Care Coordinator
You will partner with clinical care staff and providers to ensure all regulatory requirements are met to maintain patient care needs, program guidelines and/or required accreditation standards.
Act as a point of access for patients, referring providers and the Vanderbilt community.
Develop both short- and long-term goals to ensure programs' success.
Provide support to patient care team to market programs and streamline business models to allow for appropriate growth.
Establish contact with existing, newly diagnosed, potential, or referred patients by facilitating access to providers, educational materials and communication with care providers.
The key responsibilities listed are a general overview of the position and additional duties may be assigned.
