Position will provide oversight on our DEAP accreditation for outpatient diabetes education, help inpatient department achieve TJC for inpatient diabetes education as well as provide support to educators/patients and front line staff
Responsible for the planning, marketing and administrative functions of a state-wide specialty outreach clinic/extension services program. Assist with the development of specialty outreach clinics and other extension services for areas where specialty services are needed that UNMHSC physicians can provide. Ensure adherence to Hospitals and departmental policies and procedures. No patient care assignment.
Accountability:
CLINICS - Recruit providers to hold clinics, coordinate schedules and secure space at regional hospitals or clinics
COLLABORATION - Collaborate with providers, management and staff regarding operational issues of clinics, referrals and programs and referrals
FACILITATION - Facilitate referrals/consults for patient scheduling, and/or coordination of all outreach activities and post-clinic activities
OUTREACH SERVICES - Determine and acquire support personnel and equipment needed for different outreach services; arrange and coordinate travel throughout New Mexico
RELATIONS - Develop and maintain positive external relationships with all New Mexico communities; network with referring physician offices and hospitals as appropriate to ensure program success
REPORTS - Prepare weekly, monthly, quarterly, and annual reports related to program operations as well as other special reports as necessary
SCHEDULING - Prepare the daily clinic schedule utilizing the IDX system. Maintain the daily clinic schedule and prepare all statistical data. Communicate changes in the clinic schedule to supervisors, staff, support areas, and patients as needed. Perform a variety of customer service duties
BUDGET - Monitor program/project revenues and expenses for cost effectiveness of program
LIAISON - Perform as liaison between clinic staff, providers, and the insurance company or other payors to coordinate financial benefits/ coverage and prior authorizations
DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops
ADMINISTRATION - Perform various administrative functions such as monitoring expenditures and preparing reports and correspondence; may participate in a variety of research projects to develop service plans; may propose changes to program policies and procedures
CUSTOMER/STAFF RELATIONS - Establish and maintain good rapport and effective working relationships with patients, visitors, physicians and Hospital employees
OTHER DUTIES - Perform other delegated administrative support activities as deemed necessary
PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
PATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
PATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"
Education Requirements:
High School Diploma or GED equivalent
Experience Requirements:
1 year directly related experience
Education Requirements - Preferred:
MA program graduate or 2 yrs MA experience on DD Form 214
Physical Demands Requirements:
Light Work: Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. May require walking or standing to a significant degree or requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or may require working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of materials is negligible.

Tuberculosis testing is completed upon hire and additionally as required
